- Our new patient forms are on our website for you to download and fill out at your convenience. If you would rather fill them out at our office, please arrive 10 minutes early for your first appointment. If you have dental insurance be sure to bring your insurance card to your first appointment as well.
- Please give 24 hour notice when re-scheduling or cancelling an appointment.
